To make sure Scouts don’t run into any issues with participating in various outings in 2010 due to incomplete information on file, we’ll need all Scouts to submit their updated health forms.

Please download the form and submit the completed form to your den leaders. For Scouts to participate in outings, they’ll a current Health Form on file, so please submit this form as soon as you can. Thank you!
